Community Health Alliance is seeking an experienced Licensed Counselor or Social Worker (LSW, LPC, LISW or LPCC) to provide mental health counseling to individuals, families and groups in an outpatient setting. At Community Health Alliance, our Licensed Counselors and Social Workers are more than behavioral health professionals—they are an essential part of the clinical team and a vital support to providing compassionate, quality care. Through clinical expertise, therapeutic skill, and collaboration, you provide individual, group, and family therapy, conduct assessments, develop treatment plans, and implement evidence-based interventions. Using a trauma-informed, person-centered, and culturally competent approach, you support clients through crisis, treatment, and recovery while collaborating with medical, psychiatry, and case management teams to promote safety, stability, and lasting progress.

Requirements

  • Must be licensed in the State of Ohio – LSW, LPC, LISW, or LPCC
  • Bachelor or Master’s Degree in Social Work or Counseling
  • Knowledge of the DSM (Diagnostic and Statistical Manual)
  • Knowledge of and experience in diagnostic assessments, cognitive behavioral therapeutic modalities and trauma-informed care
  • Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams
  • Adept at learning and navigating electronic health record systems

Responsibilities

  • Provides individual, group and/or family therapy
  • Conducts comprehensive assessments, develops treatment plans and implements evidenced-based interventions
  • Maintains accurate and timely documentation in compliance with regulatory and ethical standards
  • Collaborates with a multidisciplinary team, including medical, psychiatry and case management
  • Utilizes a trauma-informed person-centered and culturally competent approach to client care
  • Facilitates group therapy utilizing evidence-based curricula
  • Assists clients in crisis by evaluating and providing appropriate interventions
  • Engages family and supportive others in treatment, transition and discharge planning processes
  • Evaluates clients’ progress towards goals, utilizes motivational enhancement strategies and determines appropriate interventions/modalities based on the individualized needs of each client
  • Promotes participation in self-help and recovery support groups
  • Maintains strong clinical and ethical boundaries with clients
