We are seeking a dynamic and dedicated Licensed Community Practical Nurse to join our team at the Northeast Health Center and lead the charge in providing high-quality, affordable primary care to our community. This is an exciting opportunity to manage day-to-day operations, inspire a diverse team of professionals, and ensure financial sustainability in alignment with our strategic plan. Integrated Clinical Services (ICS) is the largest Federally Qualified Health Center (FQHC) in Multnomah County, providing quality health services for people who experience barriers to accessing care. Our vision is to provide services that are integrated and connected to support optimal health, reducing wait times, and optimizing access. We are filling 3 positions with this recruitment. 1) Northeast Health Center 2) North Health Center 3) Dual North/Northeast Health Center

Requirements

  • Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
  • An Associate's degree or a Bachelor's degree in nursing from an accredited nursing program
  • Possession of, or ability to obtain, an appropriate and valid license to practice as a Licensed Practical Nurse issued by the State of Oregon by the time of hire
  • A minimum one (1) year of nursing experience
  • BLS/CPR certification required
  • Spanish / English languages (preferred)

Responsibilities

  • Panel Management Tech-Savvy Efficiency: Leverages electronic reports and scheduling systems to proactively identify and address patient needs for preventive screenings, ensuring timely care and improved health outcomes.
  • Patient-Centric Preparation: Proactively conducts pre-visit activities such as chart reviews and team huddles, ensuring each patient encounter is well-prepared and personalized for optimal care.
  • Collaborative Chronic Disease Management: Partners with providers and the care team to actively monitor chronic disease and prevention reports, initiating timely follow-ups or referrals to optimize patient health and well-being.
  • Communication Powerhouse: Seamlessly manages multiple communication channels (phone, email, in-person), ensuring clear and effective information flow across the care team and with patients, in both Spanish and English.
  • Organization & Efficiency Expert: Masters both electronic and paper systems, expertly handling inboxes, scheduling, and follow-ups to keep the team running smoothly and patients well-informed.
  • Patient-Centric Advocate: Proactively participates in reminder calls, follows up on no-shows, and ensures patients receive critical information about referrals, treatments, and medications.
  • Detail-Oriented Documenter: Maintains accurate and comprehensive patient records, utilizing SOAP notes, EPIC phrases, and clear communication to support high-quality care and collaboration.
  • Dynamic Clinical Support: Provides comprehensive clinical assistance to providers, performing a wide range of tasks from patient intake and vitals to administering medications and assisting with procedures.
  • Patient-Focused Care: Delivers compassionate and personalized care, performing focused assessments, providing patient education, and offering post-visit support to ensure optimal outcomes.
  • Skilled Multitasker: Juggles diverse responsibilities seamlessly, from performing diagnostic tests and administering treatments to completing documentation and assisting with referrals.
  • Bilingual Team Player (Preferred): Contributes to a collaborative care environment, communicating effectively with patients, providers, and colleagues in both Spanish and English to ensure exceptional patient experiences.
  • Proactive Problem-Solver: Takes initiative to troubleshoot equipment issues, ensuring minimal downtime and smooth workflow, while effectively communicating solutions and necessary repairs.
