About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) who are passionate about providing high-quality care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care and documentation. You will be an on-site provider working with residents, families, and an interdisciplinary team to address emotional, social, and psychological needs. The position involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing all documentation requirements, including staying up-to-date on insurance plans.

Requirements

• Master's degree
• Valid LCSW/LMFT/LCPC license in the state for which you are applying
• Two years of counseling and therapy experience
• Strong medical knowledge
• Understanding of documentation (CMS, Medicare, Medicaid)
• Nursing home/hospital facility experience
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
• Ability to take initiative

Responsibilities

• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support.
• Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
• Manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
• Effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
