Licensed Clinical Social Worker
About The Position
Deer Oaks Behavioral Health is seeking a part-time Licensed Clinical Social Worker (LCSW) to provide behavioral health services onsite at one or more long-term care facilities in the clinician’s local community. The role focuses on therapy, assessment, care planning, and behavioral health support for older adults and individuals with chronic medical conditions. The clinician has autonomy over their workflow and caseload while receiving administrative and clinical support from the organization. The schedule is flexible Monday through Friday, with no evenings, weekends, or on-call duties. The LCSW provides individual, family, and group therapy; conducts clinical assessments, psychosocial evaluations, screenings, and treatment planning; and supports residents experiencing a range of concerns common in long-term care settings. The role includes interdisciplinary coordination, facility partnership, and timely, compliant clinical documentation using Deer Oaks’ Geriatric Notes Tool (GNT).
Requirements
- Active, unrestricted LCSW license.
- Valid driver’s license and reliable transportation.
- Strong written, verbal, and interpersonal communication skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Comfort navigating long-term care environments.
- Commitment to timely documentation and high-quality care.

Responsibilities
- Provide individual, family, and group therapy to long-term care residents.
- Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
- Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in long-term care settings.
- Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
- Coordinate treatment and improve resident outcomes in collaboration with nursing, social services, rehabilitation, and medical providers.
- Participate in care plan discussions, facility meetings, and case collaborations as needed.
- Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
- Follow ethical, professional, and regulatory guidelines for documentation, confidentiality, and care delivery.
- Build professional relationships with facility staff and leadership and serve as a behavioral health resource for resident needs.
