About The Position

Link Rehab and Wellness is seeking a Licensed Clinical Social Worker (LCSW) to provide clinical assessments, therapy, and counseling services to geriatric individuals facing mental health challenges within two neighboring Assisted Living Facilities in Toms River, NJ, and Lakewood, NJ. The role involves developing individualized treatment plans, delivering therapeutic interventions, and collaborating with a multidisciplinary team to enhance the overall well-being of residents. The position offers a hybrid option, with a preference for on-site treatments.

Requirements

Licensed in the state of New Jersey, such as: Licensed Clinical Social Worker (LCSW) required

Responsibilities

Conduct comprehensive behavioral health assessments, including intake interviews, screenings, and evaluations to determine the mental health and/or substance use needs of clients.
Develop, implement, and revise individualized treatment plans based on the client’s goals, preferences, and clinical needs, ensuring regular updates aligned with progress and setbacks.
Provide evidence-based therapeutic interventions, including individual therapy, group therapy, and family therapy as appropriate.
Coordinate and collaborate with other healthcare professionals, including psychiatrists, medical doctors, social workers, and community organizations, to ensure comprehensive care and resource utilization.
Provide crisis intervention services, including de-escalation techniques and safety planning, for clients in acute distress or experiencing mental health crises.
Maintain accurate and timely clinical documentation, including progress notes and treatment plans in accordance with ethical, legal, and organizational standards.
Educate clients and families about mental health, coping strategies, and available resources, encouraging active participation in treatment and recovery planning.
