About The Position

Join Our Client’s Dynamic Team as a Remote Licensed Clinical Social Worker (LCSW) – Care Coordination for Long-Term Care! FlexStaff is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our clients team in a vital care coordination role—completely remote, with orientation held at our Bronx, NY location. As a Care Manager, you will utilize your specialized clinical expertise to review assessments, develop holistic care plans, and coordinate services that address all domains of patient well-being. You will work closely with the interdisciplinary team to ensure each member receives personalized, culturally competent care that promotes independence and improves health outcomes.

Requirements

  • Master’s Degree in Social Work (LMSW or LCSW) licensed in New York State.
  • Minimum of two years’ management or supervisory experience in home health, managed care, or long-term care settings.
  • At least one year working with elderly or frail populations; training provided if less experienced.
  • Excellent communication, organizational, and intercultural skills.
  • Bilingual candidates are encouraged to apply.
  • Valid NY State license and registration.
  • Ability to pass medical clearance and immunizations as per organizational policy.
  • Strong customer service orientation and a passion for improving patient outcomes.

Responsibilities

  • Conduct comprehensive review of assessments and care plans, providing guidance and coaching to field staff to ensure quality documentation and intervention delivery.
  • Participate in care planning meetings, representing your discipline and ensuring a holistic approach.
  • Collaborate with the team to coordinate services such as ADLs, IADLs, housing, transportation, and community-based resources.
  • Monitor and evaluate care activities, adjusting plans as needed to meet evolving health needs.
  • Provide ongoing training, performance feedback, and quality improvement initiatives for field staff.
  • Ensure compliance with all regulatory standards and organizational policies.
