Requirements

  • Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
  • Minimum two years of clinical counseling experience (required)
  • Valid driver’s license, reliable transportation, and auto insurance

Nice To Haves

  • Home health or telehealth experience preferred

Responsibilities

  • Deliver holistic psychosocial support and behavioral health counseling/therapy to patients.
  • Assess and coordinate community resources for patients with socioeconomic needs.
  • Develop and implement treatment plans for patients with mental health conditions or other barriers to improved health outcomes.
  • Communicate effectively with team members, providers, patients, and the community to ensure continuity of care.
  • Provide excellent customer service while collaborating with patients, families, and team members in coordinating care.
  • Maintain compliance with medical record standards, policies, and Medicare documentation guidelines.
  • Participate in organizational growth initiatives and additional responsibilities aligned with your strengths.
