Requirements
- Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
- Minimum two years of clinical counseling experience (required)
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health or telehealth experience preferred
Responsibilities
- Deliver holistic psychosocial support and behavioral health counseling/therapy to patients.
- Assess and coordinate community resources for patients with socioeconomic needs.
- Develop and implement treatment plans for patients with mental health conditions or other barriers to improved health outcomes.
- Communicate effectively with team members, providers, patients, and the community to ensure continuity of care.
- Provide excellent customer service while collaborating with patients, families, and team members in coordinating care.
- Maintain compliance with medical record standards, policies, and Medicare documentation guidelines.
- Participate in organizational growth initiatives and additional responsibilities aligned with your strengths.
