About The Position

Scotland Memorial Hospital's Transition and Navigation Services is seeking a full-time Licensed Clinical Social Worker (LCSW) to join their team. This role involves providing comprehensive psychosocial evaluations, counseling, and support to patients and their social support systems. The LCSW will collaborate with primary care providers to develop and implement care plans, address psychosocial needs, and facilitate a smooth transition across the healthcare continuum. This position requires a strong understanding of community resources and the ability to intervene in crisis situations. The LCSW will also be responsible for maintaining accurate clinical records and contributing to the development of an integrated primary care program.

Requirements

  • Minimum 1 year of professional experience in a hospital or health-related setting.
  • Expertise with data management tools.
  • Adherence to National Association of Social Workers Code of Ethics.

Responsibilities

  • Interviews patient/patient's social support system to complete an initial psychosocial evaluation/assessment.
  • Provides counseling, support, and education to patients and their social support systems, assisting them in developing healthy coping strategies and improving overall well-being.
  • Collaborates with patients and primary care providers to establish and implement a plan of care that meets identified psychosocial needs.
  • Enhances patient health by documenting patient visits, appointment attendance, and follow-up, and working to close care gaps to advance wellness.
  • Serves as a resource to the SPN regarding emotional, social, and psychosocial components of a patient's illness and its effects, assisting the care team in integrating these aspects into the plan of care.
  • Intervenes in crisis situations, applying crisis intervention skills appropriately and effectively.
  • Assesses and evaluates the patient/patient's social support system for the appropriate level of care or environment setting to meet care needs, in conjunction with the SPN team.
  • Participates in care planning meetings as requested.
  • Maintains current knowledge and researches the availability of community agencies and resources for social, emotional, or financial assistance.
  • Rounds with providers daily in the clinic to build referral relationships and conduct professional patient handoffs, ensuring clinic managers and providers are aware of daily availability and on-site hours for acute intervention.
  • Provides information and education to patients and their social support on community resources and options appropriate to the age of the patients served, in collaboration with the Clinical Care Team, and makes referrals to community agencies as needed.
  • Reports suspected cases of child & adult abuse/neglect/exploitation and serves as a liaison between the practice, patients/patient's social support system, and the Department of Social Services during evaluation/investigation.
  • Identifies and records clinical, psychosocial, and financial barriers to a smooth transition across the healthcare continuum and assists in identifying and facilitating system improvements.
  • Works with SPN leadership to identify problems, recommend solutions, and work toward resolution.
  • Develops and maintains accurate clinical records in EPIC of each referred patient, documenting according to departmental standards.
  • Collaborates with SPN leadership to build an LCSW integrated primary care program.
