Our behavioral health team works collaboratively across disciplines while giving clinicians the autonomy and flexibility to manage their caseloads and schedules. As a Licensed Clinical Social Worker (LCSW) with Impact Health PC in New Haven, CT, you will provide psychotherapy and behavioral health services to residents within skilled nursing facilities. You will manage an assigned caseload, conduct assessments, provide individual therapy, and collaborate with psychiatric providers, facility staff, and interdisciplinary care teams.

Requirements

Active Licensed Clinical Social Worker (LCSW) license in Connecticut.
2+ years of clinical experience providing mental health care to adults preferred.
Experience in skilled nursing facilities, geriatric mental health, hospitals, residential care, or psychiatric outpatient settings preferred.
Strong assessment, diagnostic, psychotherapy, and communication skills.
Ability to work independently and collaboratively as part of an interdisciplinary care team.

Responsibilities

Provide individual psychotherapy and person-centered behavioral health care.
Conduct clinical assessments and identify residents' behavioral health needs.
Manage an assigned caseload within skilled nursing facilities.
Develop and support individualized treatment plans.
Collaborate with psychiatric providers, medical staff, facility teams, and other members of the interdisciplinary care team.
Educate and support residents, families, and facility staff as appropriate.
Complete timely and accurate clinical documentation.
Maintain compliance with facility, state, and federal regulations.
