About The Position
The Clinical Social Worker provides and coordinates psychosocial and behavioral services for an assigned patient group. In collaboration with physicians, nurses, case managers, physical and occupational therapy staff, pharmacists, and other members of the medical team, the social worker assesses, plans, implements, and monitors each patient’s plan of care. In the designated setting, the social worker works with the care team to identify and advocate for patients when possible, and provides direct services, resources, and counseling to support a safe, effective, and efficient transition or discharge from the hospital or clinic. The role also provides expertise in crisis intervention and assesses vulnerable patients for potential abuse or neglect. This is a full-time, days position with a 1.0 FTE schedule, 8:00 a.m.–4:30 p.m., Monday through Friday, with some weekends. The location is Gainesville, Florida.
Requirements
- Master’s degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education.
- Recent clinical experience applicable to the designated population served.
- Valid Licensed Clinical Social Worker (LCSW) license in the State of Florida.
- Professional certification in a specialty area.
