Link Rehab and Wellness is seeking a Licensed Clinical Social Worker to join its growing startup. The role offers an opportunity to help shape programs and processes and contribute to care for the communities the organization serves. Link uses a collaborative, team-based approach and a holistic, patient-centered model focused on each individual’s needs and overall well-being. The Licensed Clinical Social Worker conducts behavioral health assessments and develops and updates individualized treatment plans. The position provides evidence-based individual, group, and family therapy as appropriate; coordinates care with healthcare professionals and community organizations; supports clients in crisis; maintains compliant clinical documentation; and educates clients and families about mental health, coping strategies, and available resources.
Requirements
- Licensed in the state of New Jersey as a Licensed Clinical Social Worker (LCSW).
Responsibilities
- Conduct comprehensive behavioral health assessments, including intake interviews, screenings, and evaluations, to determine clients’ mental health and/or substance use needs.
- Develop, implement, and revise individualized treatment plans based on clients’ goals, preferences, and clinical needs, updating plans in response to progress and setbacks.
- Provide evidence-based individual, group, and family therapy as appropriate.
- Coordinate and collaborate with healthcare professionals and community organizations to support comprehensive care and resource utilization.
- Provide crisis intervention, including de-escalation and safety planning, for clients in acute distress or experiencing mental health crises.
- Maintain accurate and timely clinical documentation, including progress notes and treatment plans, in accordance with ethical, legal, and organizational standards.
- Educate clients and families about mental health, coping strategies, and available resources, and encourage participation in treatment and recovery planning.
