About The Position

At Joyous Ventures, we are committed to providing immediate support to individuals and families experiencing homelessness or crisis. Through our dedicated Social Services Team, we help participants navigate available resources and access comprehensive care. We assist with securing healthcare services, linkages to mental health and substance abuse support, guidance with benefits enrollment, on-site counseling, therapy, support groups, and assistance with emotional well-being and relationship challenges, life skills development to foster independence, community reintegration through social activities and connections, transportation and accompaniment services, and additional support to achieve long-term housing stability. These services are crucial in promoting the well-being, empowerment, and resilience of our participants—strengthening the broader community in the process. We are seeking a compassionate and dedicated Licensed Clinical Social Worker to join our team. This role is focused on supporting residents and upholding their rights in our housing programs. You will be part of a collaborative team environment and make a meaningful impact on the lives of individuals and families striving for stability.

Requirements

  • Licensed Clinical Social Worker
  • Exceptional empathy, strong clinical and organizational skills, and a proactive, crisis-responsive approach to problem-solving.
  • Ability to foster client self-sufficiency, promote dignity, and empower individuals to lead fulfilling and independent lives.
  • Ability to provide immediate support and de-escalation during times of crisis.
  • Proficiency in maintaining comprehensive, accurate, and current participant records in electronic databases and charts - Client Management System (CMS).
  • Timely completion of Adult Initial Assessments, Discharges, and Progress Notes per Joyous Ventures and JCOD standards.
  • Experience applying harm reduction, motivational interviewing, and trauma-informed approaches.
  • Ability to encourage treatment adherence and assist in medication management support.
  • Ability to meet weekly with participants, documenting case notes, assessments, referrals, and care plans in the CMS database within 48 hours of service delivery.
  • Ability to maintain accurate records and generate reports on program activities, outcomes, and challenges.
  • Ability to participate in weekly case conferences with DHS and clinical teams.
  • Strict adherence to confidentiality and compliance with HIPAA and agency policies.
  • Willingness to attend required staff meetings, individual and group supervision, case conferences, and training sessions.
  • Ability to contribute to the development and delivery of clinical training.
  • Ability to advocate for participants' needs and the facility within the community.
  • Availability for 24/7 on-call crisis intervention, de-escalation, and support.

Responsibilities

  • Coordinate participant intake and enrollment for the Joyous Ventures program
  • Provide direct support to participants, including intake assessments, case management, and crisis intervention.
  • Facilitate weekly life skills groups (health education, self-management coaching, overdose prevention training) and lead weekly house meetings.
  • Collaborate with participants, service providers, and the Joyous Ventures team to assess needs and develop treatment/service plans.
  • Conduct needs assessments (housing, mental health, substance use, medical care, social services) and develop individualized service plans (ISP) with clients, outlining SMART goals.
  • Connect participants to community-based services (behavioral health, substance use treatment, housing, food assistance, and transportation).
  • Cultivate a supportive community atmosphere and promptly address participant concerns.
  • Maintain comprehensive, accurate, and current participant records in electronic databases and charts - Client Management System (CMS), ensuring timely completion of Adult Initial Assessments, Discharges, and Progress Notes per Joyous Ventures and JCOD standards.
