The Behavioral Health Counselor is a licensed (or conditionally licensed) therapist providing outpatient psychotherapy treatment to patients including clinical assessment, diagnosis, treatment, ongoing risk assessment and crisis management. The role involves addressing behavior health issues from mental health to substance abuse for patients across the lifespan, documenting patient treatments via EMR computer software, and obtaining/creating patient's psychosocial history. The practitioner will also function independently to perform age-appropriate history and physical for complex acute, critical and chronically ill patients, provide clinical consultation to medical staff, and maintain clinical availability to deliver solution-focused and relationship-based therapy. Responsibilities include addressing family and relationship issues, understanding and observing role boundaries, monitoring intervention effectiveness, facilitating patient transitions within and between healthcare settings, and collaborating with multidisciplinary team members by making appropriate referrals. Documentation of care in the electronic health record and ensuring efficient, smooth, and prompt delivery of health care services are also key aspects of the role. The practitioner may also practice in the community, including home visits, school, and other social environments.
