About The Position

Community Health Center of the North Country seeks a compassionate and dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our Federally Qualified Health Center’s Behavioral Health Department. Requirements

  • Master’s Degree in Social Work required.
  • Current, registered NYS LMSW license required.
  • At least 3 years of post-M.S.W. supervised experience in diagnosis, psychotherapy, and assessment-based treatment planning with a current, registered NYS LCSW license preferred
  • Experience with electronic medical records is preferred.
  • Strong organizational and planning skills.
  • Ability to maintain good working relationships with others.
  • Exhibits a high level of professionalism and confidentiality in dealing with issues.
  • Sedentary work; ability to occasionally lift and carry up to 25 lbs.
  • Ability to sit for long periods.
  • Ability to tolerate exposure to a computer monitor/screen.
  • Ability to tolerate the extended use of a computer keyboard.

Nice To Haves

  • Current, registered NYS LCSW license preferred

Responsibilities

  • Perform psycho/social assessment.
  • Develop a therapeutic counseling regimen to meet the individual's needs based on assessment and input from caregivers/family.
  • Coordinate treatment plan, referring individuals as necessary for psychiatric consult and prescription.
  • Provide ongoing counseling as indicated and follow up if a psychiatric consultation is made.
  • Maintain patient records in accordance with agency policies and procedures.
  • Provide ongoing supervision of MSWs, as necessary and appropriate.
  • Perform other duties as requested by the supervisor.
