About The Position

Community Health Center of the North Country seeks a compassionate and dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our Federally Qualified Health Center’s Behavioral Health Department. Requirements

Master’s Degree in Social Work required.
Current, registered NYS LMSW license required.
At least 3 years of post-M.S.W. supervised experience in diagnosis, psychotherapy, and assessment-based treatment planning with a current, registered NYS LCSW license preferred
Experience with electronic medical records is preferred.
Strong organizational and planning skills.
Ability to maintain good working relationships with others.
Exhibits a high level of professionalism and confidentiality in dealing with issues.
Sedentary work; ability to occasionally lift and carry up to 25 lbs.
Ability to sit for long periods.
Ability to tolerate exposure to a computer monitor/screen.
Ability to tolerate the extended use of a computer keyboard.

Nice To Haves

Current, registered NYS LCSW license preferred

Responsibilities

Perform psycho/social assessment.
Develop a therapeutic counseling regimen to meet the individual's needs based on assessment and input from caregivers/family.
Coordinate treatment plan, referring individuals as necessary for psychiatric consult and prescription.
Provide ongoing counseling as indicated and follow up if a psychiatric consultation is made.
Maintain patient records in accordance with agency policies and procedures.
Provide ongoing supervision of MSWs, as necessary and appropriate.
Perform other duties as requested by the supervisor.
