About The Position

The Licensed Clinical Social Worker (LCSW) is responsible for making clinical decisions and exercising judgment to ensure the best possible care is provided within available resources. This role involves providing counseling services, with a focus on individual and family issues and decisions impacting patient and family health. The LCSW will work within a primary care model of brief therapy, facilitating seamless transitions to community resources as needed. A strong understanding of community resources, particularly in crisis services, mental health, and long-term care, is essential. The position also involves assisting referring physicians and healthcare providers with referrals to secondary and tertiary health care centers. The LCSW will utilize knowledge and skills in group work, including cognitive behavior therapy and mind over mood. This role requires active participation in data management and quality improvement initiatives, focusing on patient outcomes and communicating findings for potential improvements. The LCSW will act as an advocate for patients and their families, liaise with physicians and other team members, and educate family medicine residents and other healthcare professional students on the role of social work and psychosocial issues. Effective application of social work standards of practice and therapy processes for patient care assessment, planning, implementation, and evaluation is expected. Maintaining continuing education, collaborating with healthcare team members, and acting as a professional role model are key aspects of this position. Confidentiality in the workplace is paramount.

Requirements

  • Master's Degree preferred
  • Registered with the Louisiana State Board of Social Work Examiners
  • Previous related patient care experience, specifically in a primary care setting preferred
  • Minimum 2 years experience preferred
  • Experience in solution focused brief therapy, cognitive behavior therapy, motivational interviewing, and mindfulness
  • Ability to operate with highest level of confidentiality and discretion
  • Strong conflict resolution
  • Exceptional oral, written, and presentation skills
  • Strong interpersonal skills
  • Group facilitation skills
  • In depth knowledge of chronic illness care, including both clinical and supportive care
  • In depth knowledge of the local health program delivery and community-based services
  • Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population
  • Ability to work effectively with all members of the health care team
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment

Responsibilities

  • Making clinical decisions and exercising judgment to ensure best possible care is provided within available resources.
  • Providing counseling services, with particular regard to individual and family issues and decisions that impact health of the patient and family.
  • Working within primary care model of brief therapy, ensuring seamless transition to community resources as appropriate.
  • Assisting the referring physician/health care providers with referrals to secondary and tertiary health care centers.
  • Participating in data management and measurement of quality improvement indicators of patient outcomes.
  • Communicating results of measurements that are easy to use and understand, looking for patterns that could assist with improvements.
  • Acting as advocate for patients and their families in matters affecting health.
  • Liaising with physicians and other team members as required.
  • Teaching family medicine residents and other health care professional students on the role of the social worker and psycho-social issues facing patients and families.
  • Effectively using the social work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient.
  • Maintaining own continuing education to enhance professional knowledge and growth.
  • Participating and collaborating with other family health team members and health care providers in provision of patient care.
  • Acting as a professional and knowledgeable role model or mentor to colleagues and students.
  • Maintaining confidentiality in the workplace within the community.
