Licensed Clinical Social Worker - LCSW
About The Position
Provides psychotherapeutic services, case management, and support for patients in an ambulatory care setting in person and/or telephonically. Provides assessment and diagnosis for psychological, emotional or behavioral disorders. Makes recommendations and implements appropriate treatments plans. Monitors progress through follow-up sessions and/or phone consultations. Serves as a member of interdisciplinary team consulting with physicians, APPs, nurses, psychiatrists, and medical staff. Assures the deliverance of quality treatment to patients utilizing best practices and standards of care. Provides appropriate coordination of resources, referrals, and support in an effort to meet the total needs of the patient.
Requirements
- Master’s Degree in Social Work (MSW)
- Active, unrestricted Licensed Clinical Social Worker (LCSW) licensure
Nice To Haves
- Previous experience in the continuity of patient care planning
- Previous experience with crisis intervention
- Bilingual in Spanish
