About The Position

Community Hope is looking for a full-time Licensed Case Manager for our Hope for Veterans Program. This role involves providing assessment, diagnosis, individual and group psychotherapy, and support services to veterans enrolled in the program. The position supports veterans experiencing various mental health challenges, grief, PTSD, behavioral issues, relationship challenges, or substance use. The Case Manager will also develop Individual Recovery Plans (IRPs) in compliance with VA Grant & Per Diem guidelines, participate in clinical on-call rotations, develop recovery support groups, transport veterans as needed, provide medication education, and maintain accurate clinical documentation while upholding veteran confidentiality. Teamwork is essential, including facilitating groups, participating in program activities, and covering co-workers' duties.

Requirements

  • Master’s degree in non-profit or human services management, mental health, substance abuse, or a related discipline.
  • Knowledge of veterans’ issues, mental illness, addiction, and/or homelessness required.
  • Must possess a current NJ LSW, LAC, LCSW or LPC licensure.
  • Must possess a valid driver’s license, with a good driving record.

Responsibilities

  • Provide assessment, diagnosis, individual psychotherapy, group psychotherapy, and support services for veterans enrolled in the Hope for Veterans Program.
  • Provide direct services, including assessment, evaluation, recovery planning, case conferences, referrals, family meetings, group and individual psychotherapy, veteran advocacy, and linkage to community resources.
  • Develop Individual Recovery Plans (IRPs) in compliance with VA Grant & Per Diem guidelines.
  • Support veterans experiencing mental health symptoms, grief, PTSD, behavioral issues, relationship challenges, or substance use.
  • Participate in Clinical On-Call rotation and respond to emergent employee and veteran needs.
  • Develop recovery support groups and activities to meet veteran needs.
  • Transport veterans, as needed, in agency vehicles.
  • Provide medication education to promote safety and independence.
  • Maintain accurate and timely clinical documentation while upholding veteran confidentiality.
  • Function as a team member: facilitate groups, participate in program activities, share relevant information, and cover co-workers’ duties as needed.
