About The Position

The Case Manager for the Acute Rehabilitation (ARU) unit coordinates and manages patient care to ensure high‑quality outcomes, optimal resource utilization, and compliance with regulatory and external review requirements. This role provides comprehensive assessment, care planning, coordination, and ongoing evaluation of individual patient needs while honoring patient preferences and supporting continuity of care. Serving as a patient advocate, the Case Manager facilitates discharge planning, addresses holistic needs, and communicates effectively with patients, families, and the interdisciplinary team. Through the integration of case management, utilization review, and discharge planning functions, this position enhances patient satisfaction, promotes cost‑effective care, and supports the hospital’s mission, vision, and values.

Requirements

  • Current Registered Nurse (RN) or Social Work licensure (LMSW, LCSW, LICSW) or Healthcare professional licensure as Physical Therapist (PT), Speech Language Pathologist (SLP) or Occupational Therapist (OT).
  • Minimum of two (2) years social work or case management experience in an inpatient setting highly preferred
  • Acute/rehabilitation hospital experience preferred.

Nice To Haves

  • Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
