Requirements

  • Current licensure as a Registered Nurse, Social Worker, or other licensed healthcare professional (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist).
  • Proficiency with basic computer applications (e.g., Excel, Word, Outlook, PowerPoint).
  • Excellent organizational, time management, and analytical skills, with the ability to work independently.
  • Collaborative mindset with a strong team-oriented approach.
  • Commitment to maintaining confidentiality and professional standards.

Nice To Haves

  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, ARN, RN-BC).
  • Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute or rehabilitation hospital experience strongly preferred.
  • Strong written and verbal communication skills in English; additional languages are a plus.

Responsibilities

  • Coordinate patient care to ensure effective resource utilization, quality service delivery, and compliance with regulatory requirements.
  • Conduct comprehensive assessments, develop and implement care plans, and evaluate patient progress to support individualized needs and preferences.
  • Facilitate care coordination and communication across the interdisciplinary team, patients, and families.
  • Lead discharge planning efforts to promote smooth transitions, continuity of care, and cost-effective outcomes.
  • Advocate for patients by addressing their clinical, emotional, and social needs.
  • Support utilization review and case management functions to enhance patient outcomes and satisfaction.
  • Promote and uphold the hospital’s mission, vision, and values.
