Manage a client caseload within the outpatient behavioral health clinic. Conduct intake assessments, including biopsychosocial evaluations. Collaboratively develop, implement, and update individualized treatment plans and goals. Provide individual, family, and group counseling using evidence-based and culturally responsive practices. Facilitate therapeutic groups and psychoeducation sessions. Document all clinical encounters, treatment progress, and case management notes in the electronic health record in a timely and compliant manner. Provide crisis intervention and stabilization support appropriate to an outpatient setting. Coordinate care and make referrals to psychiatry, primary care, urgent care, social services, and other community resources as needed. Provide and receive warm handoffs between urgent care, primary care, and behavioral health services within Transformation Health Network to support continuity of care and whole-person treatment. Collaborate with schools, courts, probation/parole, and other external partners when client consent or court order applies. Attending team meetings, interdisciplinary case conferences, and clinical supervision. Engage in regular training and professional development to support high-quality, whole-person care. Perform other related duties as assigned or needed to support clinic operations.
