About The Position

Provides direct service in a GLE/SIE residential setting, helping individuals overcome barriers and prepare for independent living. Implements treatment plan support interventions as coached by the Team LPHA Clinician. Assumes administrative officer roles to support the Program Director and liaisons with families as needed. Supports individuals in obtaining and understanding medical and psychiatric care, administers medications according to program policies, and provides crisis interventions. Responsible for physical site maintenance, ensuring a safe, comfortable, and clean environment.

Requirements

  • A high school diploma or equivalent is required.
  • One to two years of job-specific experience in the human services field preferred.
  • Driving is a requirement for this position.
  • Must possess and maintain adequate insurance and a safe driving record if using a personal vehicle.
  • A valid driver's license must be presented at the time of employment.
  • Must be at least 21 years of age.
  • Must have maintained a valid US driver's license for at least six months.
  • Must be able to pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb.
  • Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift.
  • Ability to operate a computer and other office equipment.
  • Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Ability to assist residents with tasks of daily living.
  • Ability to remain in a stationary position 50% of the time as needed.
  • Ability to bend, reach, file, sit, stand, and move around the facility.
  • Ability to speak, hear, and communicate with residents, staff, and external representatives.
  • Specific vision abilities required: close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • CPR required within two weeks of hire.
  • First Aid required within two weeks of hire.
  • MAP required within 90 days of hire.

Nice To Haves

  • Associate’s degree in a human service discipline preferred.
  • One year as a residential counselor in a setting working with Persons Served (PS) facing mental health challenges preferred.

Responsibilities

  • Assists the Program Director in orienting new residents.
  • Collaborates with Integrated Team members to address resident needs and implements treatment plan interventions.
  • Performs day-to-day assignments as directed by the Program Director.
  • Uses recovery-oriented, person-centered, harm-reduction, trauma-informed, culturally competent, and strengths-based approaches with residents.
  • Adheres to safety, maintenance, Human Rights, and other operational policies and procedures.
  • Assumes roles of Medication Officer, Safety Officer, Human Rights Officer as assigned by the Program Director.
  • Schedules and facilitates weekly group meetings with residents, including menu input.
  • Performs primary grocery shopping weekly or as needed for the residence.
  • Complies with physical site licensing standards and regulations.
  • Stores, administers, documents, and disposes of resident medication consistent with Medication Administration Program (MAP) requirements and company policy.
  • Teaches residents medication management.
  • Creates opportunities for residents to participate in the smooth operation of the residence, including meal preparation and physical site maintenance.
  • Encourages social interaction among residents and helps create a collaborative environment.
  • Serves as a resource for neighbors.
  • Helps residents prepare meals.
  • Acts as an advocate for residents.
  • Participates in trainings, supervision, and staff meetings.
  • Completes all required documentation.
  • Contributes to program's quality improvement goals and objectives.
  • Helps residents use crisis intervention techniques when necessary and accesses on-call back up when needed.
  • Supports the provision of evidence-based practices and best practices to residents.
  • Completes Infection Control Inventory for the residence.
  • Assists the Program Director with organization of surveillance testing of staff and residents as needed.
  • Completes incident documentation as needed.
  • Performs physical site maintenance activities following the maintenance schedule.
  • Performs other related duties as assigned.
