About The Position

Under the supervision of the Senior Medical Services Manager, the Registered Nurse (RN) is responsible for carrying out functions within the RN scope of practice and acts as the RN Lead. This role involves providing direct patient care, performing patient triage and assessments, coordinating services between various medical providers, completing medication reconciliation, and overseeing complex, high-risk patients. The position also supports the Senior Medical Services Manager in clinical operations, provides oversight and guidance to Registered Nurses, and acts as a clinical resource for staff. Additionally, the Lead RN participates in quality improvement, population health initiatives, and special projects to support the mission of Roots Community Health Center.

Requirements

  • Valid, current California license as a registered nurse
  • Current AHA CPR certification
  • A Bachelor’s Degree in Nursing and a California Public Health Nurse certificate and a minimum of two (2) years’ experience in nursing

Responsibilities

  • Provide direct patient care and education under established standing orders and standardized procedures, including conducting nurse visits for preventive care, chronic disease monitoring, acute concerns, sensitive services etc.
  • Perform patient triage and nursing assessments to determine patient needs, prioritize care, and coordinate timely provider involvement when clinically indicated.
  • Coordinate services between Physicians, specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients.
  • Complete medication reconciliation and review during patient encounters, ensuring the accuracy of medication lists and the patient's understanding of medication use.
  • Provide oversight of a population of complex, high-risk patients, including monitoring identification of those eligible for enhanced care management, care plan completion, and patient outcomes.
  • Attend departmental and organizational meetings as required.
  • Actively participate in integrated team care management rounds.
  • Document patient care activities accurately and in a timely manner within the Electronic Health Record in accordance with organizational policies.
  • Support the Senior Medical Services Manager in the coordination and oversight of clinical operations to ensure efficient, high-quality patient care delivery.
  • Provide support with Registered Nurse (RN) oversight, including assisting with clinical guidance, workflow support, and reinforcement of nursing standards and protocols as directed by the Senior Clinical Services Manager.
  • Provide operational support in addressing day-to-day clinical issues that arise in the clinic, including assisting with patient flow, clinical problem-solving, and coordination with interdisciplinary team members.
  • Serve as a clinical resource for staff and assist in facilitating interdepartmental communication between nursing staff, departments, providers, and leadership to support coordinated care delivery.
  • Participate in special projects, clinical initiatives, and other administrative duties that support the overall mission and operations of Roots Community Health Center.
  • Participate in quality improvement and population health initiatives, including outreach and engagement efforts to support preventive care and chronic disease management
