About The Position

The Psychosocial Rehabilitation (PSR) Program at Park Center builds community in east Nashville with adults diagnosed with mental Illnesses. All PSR staff share in assisting members to live vocationally productive and socially satisfying lives. The program participants are perceived as “members” rather than “clients” or “patients”, and they work to maintain an informal setting to reduce the psychological distance between staff and members. Staff encourage members to assume productive roles both within the psychosocial clubhouse facility, and in the broader community. Coordinators provide members support through working to maintain a flexible, varied program of vocational, educational, and recreational activities. They are actively involved with the development of supported employment pathways and placements in the community and actively involved with their sustainability. The Lead Recovery Coordinator aids in support and training to Recovery Coordinatorss in the PSR program. Oversees scheduling of the programs activities and groups, orients new members, leads groups, and works with a small caseload of no more than 10 members. Assists with transportation scheduling and reconciliation of new and existing members to ensure program attendance. Fills in for staff and supervisor with they are absent providing coverage needed.

Requirements

  • Bachelor’s Degree from an accredited college in social work, sociology, or another related field.
  • Three (3) years minimum of experience in a mental health or social service setting.
  • Requires an experienced candidate with intermediate to advanced clinical skills.

Responsibilities

  • Meets with new staff to assist them in learning the electronic database, and provides ongoing assistance as required.
  • Trains staff in Park Center’s philosophy and systems, in collaboration with the PSR SR Manager.
  • Fills in for staff or supervisor as needed.
  • Assists with arranging transportation of members to and from the program.
  • Assists Recovery Coaches with members who may need specialized assistance or referral to outside agencies.
  • Meets with staff members on a regular basis to create the monthly schedule and tweak it as necessary.
  • Provides guidance to staff on the development of specific groups and activities.
  • Meets with orientation members and ensures that they have signed all necessary documentation.
  • Works with supervisor to ensure that transportation is scheduled.
  • Checks in with orientation members for their first week and informs supervisor if the member does not attend.
  • Notifies the PSR SR Manager of any new admissions to PSR programs on the day of orientation.
  • Maintains a small caseload of no more than 10 members.
  • Meets with members assigned to the recovery team, assisting them in identifying recovery tracks, goals, and objectives to meet their goals.
  • Works collaboratively with each member on the team to develop a daily schedule as reflected on the daily schedule cards.
  • Meets with assigned members as a group minimally two times a day – in a goal discussion group in the morning and a wrap-up meeting in the afternoon. May also meet with members throughout the day.
  • Touches base with members throughout each day encouraging them to attend the activities/groups they signed up for, and to ensuring that their day is progressing well.
  • Calls assigned members who are not attending at least weekly.
  • Leads groups each week as assigned.
  • Maintains attendance sheets for the group daily.
  • Develops a syllabus/binder for groups.
  • Completes all group and activity notes according to policy.
  • Works with members to complete recovery plans every three months.
  • Maintains all documentation as required.
  • Ensures that all information in the member record is signed as required, of high quality, and is clearly understood by members, other staff, and monitoring sources.
  • Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds.
  • Take deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.
  • Promotes culturally responsive services and continuous competency development.
  • Acts as a liaison between transportation carriers and Park Center.
  • Ensures daily attendance is recorded accurately.
  • Meets with referral sources as needed.
  • Attends all required meetings.
  • Participates on agency-wide committees.
  • Completes all required training.
  • Maintains confidentiality and works in a professional manner.
