About The Position

Cornerstone Family Healthcare is actively recruiting for a Lead LPN to join our growing team in Newburgh, NY. This role provides oversight of the nursing and clinical care provided within the Internal Medicine, Pediatrics and Specialty departments at Cornerstone’s KFP location. Requirements

  • Successful completion of accredited school of Nursing
  • Minimum - NYS Education Department Registered Nurse or Licensed Practical Nurse License
  • Current BLS Certification

Responsibilities

  • Provide supervision to assigned direct reports as well as oversight to clinical staff.
  • Responsible for meeting with each site’s Clinical Coordinators and/or nursing team to provide supervision on a weekly/bi-weekly basis. In-person or video meeting.
  • Works with Clinical Coordinators and operational team to ensure appropriate staffing.
  • Ensure that Nursing and support staff remain current with screening duties and policies as per CORNERSTONE policies and procedures.
  • Assist Nursing and support staff as needed by participating in Cornerstone’s Care Team Model including but not limited to: Participating in Pre-Visit Planning, Direct population management activities for assigned patient panels, Participate in quality improvement activities within the care team and department, Assist providers with in-office procedures and exams, Administer medications, treatments and vaccines in accordance to provider orders and scope of practice.
  • Participates within the multidisciplinary team to provide evidence-based care for chronic disease management.
  • Provides an environment that engages staff and others in decision making.
  • Cultivates a cohesive environment by addressing and managing conflict in a constructive and productive manner.
  • Strictly adhere to safe nursing practice including but not limited to Infection Control, National Patient Safety Goals, Medication Management and Waived Testing set forth by CORNERSTONE policy and incompliance with Joint Commission and OSHA.
  • Ensure staff participation and understanding in CORNERSTONE’s measures including but not limited to PCMH and the care team model, meaningful use, UDS, HEDIS, DSRIP, Infection Control, and Joint Commission standards
  • Provide specific nursing skills as indicated by medical provider and in accordance with NYS Licensed Professional Nurse scope of practice.
  • Manage patient flow based on the volume of patients in the department and the needs of each provider. Coordinate with departments and Practice Managers for staff needs.
  • Perform random chart reviews and routinely monitor department flow for efficiency and make changes when needed.
  • Monitor staff adherence to the patient’s experience/satisfaction and provide feedback or additional training when necessary.
  • In concert with department chief, participates in CQI projects and monitor progress on quarterly basis. Develop nurse/MA interventions directed at CQI project and develop staff education as necessary.
  • Ensure monthly/weekly safety and clinical inspections are completed and submitted to appropriate party. Conduct random unannounced unit inspections to ensure adherence.
  • Ensure that all pharmaceuticals, both stock and samples, appear on approved medication list, are inventoried monthly to check for expiration dates and recalls. High Alert and look alike/sound alike medications are labeled as such
  • Ensure the DEPARTMENT PROXY in our electronic medical record is assigned and addressed in a timely manner and is free of outstanding phone notes/messages, orders concerning laboratory follow up from the Providers.
  • Appropriately maintain or assign all needed departmental reporting.
  • Assists Clinical Coordinators in yearly staff education, training, competency, and evaluations as needed.
  • Responsible for coordinating care and providing patient education based upon their needs.
  • Attend and participate in monthly department, All Staff, and other required meetings. Facilitate department staff huddles as needed.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the CORNERSTONE Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of CORNERSTONE including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other duties as assigned.
