About The Position

Provides leadership in supervision and coordination of therapy delivery of patient care through and as an active member of the rehabilitation team. Plans, organizes and directs activities for the treatment of patients. Coordinates, establishes and provides discipline specific clinical, regulatory and billing education to other therapists, nurses, physicians and others as needed.

  • An active license in the state of practice
  • 1-3 years clinical experience

Responsibilities

  • Provides care and treatment of assigned patients/clients/students/residents.
  • Assesses patient needs, deficits, and strengths and formulates appropriate treatment plan.
  • Develops and manages plan of care identifying specific goals and activities for each patient based on evaluation findings.
  • Addresses goals in a functional and measurable manner.
  • Coordinates the day to day activities of the staff and maintains productivity goals established.
  • Observes patient’s actions and reactions to prescribed treatment, evaluating progress.
  • Communicates observations, facts and comments with interdisciplinary team members individually and through team conferences.
  • Communicates with physicians regarding patient status.
  • Maintains appropriate documentation, including but not limited to evaluation reports, treatment plans, treatment notes and discharge reports.
  • Creates and reviews documentation, revising as necessary to provide therapeutic interventions.
  • Provides effective training and education to patient, family members and caregivers.
  • Assesses Knowledge of discipline specific billing regulations, documentation requirements, and treatment specific guidelines across all therapy service areas.
  • Administers and performs therapeutic interventions with patients ensuring proper charges/billing information is submitted.
  • Actively participates in the evaluation of work quality and outcomes, both individually and as a part of the team.
  • Monitors, evaluates and guides improvements in discipline specific therapy outcomes.
  • Collaborates closely with other Therapy Managers and Clinical Leads to maintain communication between groups, maintain consistency of practices, and share resources as needed.
  • Evaluates staff therapist competency and creates/directs ongoing staff competency and quality of care in collaboration with Therapy Managers and Executive Director.
  • Responsible for discipline specific staff training and ongoing education.
  • Coordinates and directs discipline specific Quality Assurance initiatives.
