About The Position

PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Housing & Landlord Partnership team as the Lead Housing Specialist. Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. The Lead Housing Specialist will assist in breaking the cycle of homelessness by supporting clients from street to housing. The Lead Housing Specialist is a seasoned social services professional who collaborates with the program manager(s) to support the needs of the client and provide direct support to the team as needed. The Lead Housing Specialist maintains a direct client caseload and takes on additional duties to support the operations of the program and the Program Manager.

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete the following as a condition of hire: Tuberculosis Test, Background Screening, Drug Test
  • Have reliable transportation
  • A valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

Nice To Haves

  • Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

Responsibilities

  • Monitor and manage the housing team's customer relationship management (CRM) system, including providing biweekly availability reports to PATH programs.
  • Under the supervision of the program manager, provide leadership & support to the team, ensuring continuity of care and services between PATH's agency resources and the partnering agencies.
  • Familiarity with organizational policies and standards to uphold amongst the team.
  • Collaborate with intradepartmental and interdepartmental peers to meet client care needs and programmatic goals.
  • Under the supervision of the program manager, effectively communicates project expectations to team members.
