About The Position

The Lead Direct Support Professional (LDSP) supports the Site Manager to ensure the smooth and cohesive operation of the program. Ensures, through support and advocacy the consumer’s individual needs, work environment, and is the mentor to all team members. Serves as the point-of-contact in the manager’s absence. Ensures that consumer’s individual needs at met using a person-centered approach; supports efforts to ensure that all regulatory standards are met. Delivers services that achieve quality, utilization, outcome, and financial sustainability with a high level of individual and stakeholder satisfaction.

Requirements

  • Must be at least 18 years old.
  • High school diploma or GED equivalent required.
  • Minimum 2 years’ experience working with persons with disabilities.
  • Possess a valid driver’s license & clean driving record.
  • Strong verbal and written communication skills, with proficiency in Microsoft Office Suite, including Outlook, Excel, and Word.

Responsibilities

  • Takes a lead role in adhering to all regulatory compliance, and training requirements are maintained with 100% compliance, follows ensures all policies and procedures follow licensing, regulatory, accreditation and internal standards.
  • Collaborates with internal and external teams, regulatory agencies, and stakeholders with professionalism in accordance with Easterseals Core Values
  • Adheres to the consumer’s plan of care in all areas of adult daily living: providing direct supports in the community, facility, or in their own home,
  • Documents Unusual Incident Reports (UIR’s) and submits to compliance and DDD/UPDOC within the required timeframes.
  • Participates in audits, inspections, and investigations.
  • Assists management with continuity of care.
  • Ensures all documentation is entered into Electronic Healthcare System (EHR).
  • Delivers quality services that achieve quality, utilization, outcome, and financial sustainability with a high level of individual and stakeholder satisfaction.
  • Accountable for individual funds and provides hands-on training to DSPs in the manager’s absence (second key holder).
  • Promotes safety and welfare of self and others.
  • Supports and interacts with persons served throughout all scheduled shifts.
  • Responds timely to all individuals and stakeholder questions, concerns, and complaints and relays information to theSite Manager to ensure all communication is clear and addressed.
  • Leads the process of monthly purging of all documentation to be scanned into the EHR system.
  • Participates in mandatory staff meetings and communicates new ideas and knowledge that improves services and operations.
  • Utilizes the application of technology within operations and services to enhance quality, increase outcomes and lower costs.
  • Advocates for rights and dignities of persons served that promote their inclusion into community life.
  • Intervenes during crisis as trained and able.
