About The Position

MYEP is hiring a Skilled Care Lead Direct Support Professional to provide direct support, skilled care assistance, and on-shift leadership within our Residential Program. This position plays a key role in ensuring high-quality, person-centered services for adults with disabilities while supporting the staff team who work in the home. The Skilled Care LDSP helps oversee the site, the people who live there, and the staff who work there. This role combines hands-on direct care with leadership responsibilities, including training, coaching, scheduling support, documentation oversight, quality checks, and regular communication with supervisors. This position is essential to maintaining consistency, accountability, strong team performance, and excellent care for the individuals served. The Skilled Care LDSP is a leadership position within the Residential Program. This role helps ensure the home runs smoothly, staff are supported and held accountable, documentation and care expectations are met, and the individuals served receive consistent, high-quality support. Requirements

  • High school diploma or equivalent required
  • Experience in human services, residential services, or direct support preferred
  • Valid driver’s license and clean driving record required
  • Ability to provide hands-on personal care and skilled care supports
  • Ability to lead by example and support a team environment
  • Strong communication, organization, and problem-solving skills
  • Ability to work independently and make sound decisions
  • Comfort using documentation systems, scheduling systems, and other agency technology
  • Commitment to person-centered services, confidentiality, and professional boundaries

Responsibilities

  • Provide direct support to adults with disabilities in a residential setting
  • Provide on-shift leadership, guidance, and coaching to staff
  • Train, support, and supervise staff working at the site
  • Assist with onboarding and hands-on training of new staff
  • Oversee daily routines, service delivery, and staff follow-through
  • Support scheduling needs and help ensure appropriate staffing coverage
  • Assist with personal care, skilled care supports, and daily living skills
  • Administer medications following agency protocols
  • Monitor and report changes in health, behavior, mood, or support needs
  • Ensure documentation is completed accurately and timely
  • Review documentation for accuracy, completeness, and compliance
  • Conduct quality checks and help ensure agency standards are met
  • Assist with community outings, appointments, and transportation
  • Support skill development, independence, and quality of life
  • Communicate regularly with supervisors regarding service delivery, staffing needs, concerns, and site operations
  • Maintain confidentiality and follow HIPAA guidelines
  • Provide hands-on personal care, including bathing, showering, grooming, and oral hygiene
  • Provide hands-on personal care, including incontinence care and toileting routines
  • Provide hands-on personal care, including dressing and personal grooming assistance
  • Provide hands-on personal care, including transfers, manual lifts, and overhead lifts
  • Provide hands-on personal care, including range of motion and physical support exercises
  • Provide hands-on personal care, including meal preparation and assistance with eating
  • Provide hands-on personal care, including monitoring food, water, and medication intake
  • Provide hands-on personal care, including mobility assistance with wheelchairs, walkers, and gait belts
