About The Position

The Lead Coordinator - Social Work position at Cincinnati Children's, within the PS Network - OhioRise department, is a full-time role with a weekly schedule of 40 hours. This position is responsible for providing comprehensive social work services, including psychosocial assessments and interventions, interdisciplinary collaboration, and resource development. The role also involves leadership responsibilities such as providing work direction, mentoring, and coordinating staff activities to ensure high-quality patient care and compliance with regulations.

Requirements

  • Master's degree in Social Work or Counseling
  • 5+ years of work experience in a related job discipline.
  • Current independent Social Work (LISW) or LPCC license to practice in the State of Ohio OR LSW with LISW within 1 year of hire.

Responsibilities

  • Consistently integrates age-specific and culturally inclusive concepts into patient care, considering both chronological age and developmental functioning.
  • Works for system improvement, promoting patient/family/caregiver well-being.
  • Identifies and advocates for the needs of patients/families/caregivers within the healthcare system and community.
  • Acts as a liaison between the patient/family/caregiver, the medical team, and outside agencies.
  • Mediates within the medical team on behalf of the patient/family/caregiver as needed.
  • Empowers patients/families/caregivers to problem-solve by exploring options of care and alternative plans.
  • Facilitates communication and coordination between healthcare team members, involving the patient/family/caregiver in decision-making.
  • Conducts comprehensive psychosocial assessments to identify patient/family health care needs, strengths, challenges, and opportunities.
  • Evaluates patient/family/caregiver safety concerns and intervenes appropriately to reduce risk.
  • Reports suspicions of abuse and neglect to legally mandated authorities.
  • Utilizes psychosocial assessments to establish treatment interventions for comprehensive plans of care.
  • Provides psychosocial services as identified in the patient's comprehensive plan of care.
  • Offers supportive counseling to improve emotional coping and adherence to treatment plans.
  • Collaborates with referral sources and healthcare team members.
  • Educates patients/families/caregivers and healthcare team members about treatment options.
  • Provides self-management support to high-risk/complex patients/families.
  • Provides groups and programs for special populations as needed.
  • Provides anticipatory guidance and planning to reduce psychosocial barriers to discharge.
  • Coordinates and facilitates family-centered care team meetings.
  • Provides crisis assessment and interventions to restore equilibrium.
  • Develops resource networks, excels at resource utilization, and acts as a resource to peers.
  • Identifies needs, develops programs, and evaluates outcomes for special populations.
  • Facilitates collaboration with community agencies.
  • Advocates for patient populations on a systems level by developing programs and protocols.
  • Serves as a recognized expert to the healthcare team and/or community on social work practice.
  • Provides work direction, leadership, mentoring, and coaching to assigned personnel.
  • Participates in the recruitment and selection process for candidates.
  • Coordinates work schedules for all clinical staff.
  • Coordinates the clinical activities of clinical staff and meets with them routinely.
  • Provides input for employee annual performance evaluations.
  • Participates in the development of long-term and short-term goals for their location and employees.
  • Works as a liaison between school districts and the division.
  • Attends monthly and quarterly meetings for school districts involving themselves and supervised staff.
  • Ensures clinical practice conforms to local, state, and federal regulations and accrediting standards.
  • Ensures clinical competency of all clinical staff.
  • Responds to, documents, and analyzes complaints associated with therapy services.
  • Maintains minimum continuing education required by the State licensing board.
  • Possesses sound knowledge of procedures and regulations specific to the assessment and care of infants, children, and adolescents.
  • Complies with established Cincinnati Children’s and professional standards.
  • Complies with policies and procedures protecting patient rights for informed consent and handling of confidential information.
  • Reviews timeliness, accuracy, availability, and security of information.
  • Attends required Cincinnati Children’s training sessions.
  • Complies with Infection Control policies and procedures.
  • Provides services in a manner to be regularly and conveniently available to meet the needs of Cincinnati Children’s patients, families, and employees.
