About The Position

The Lead Coordinator- Social Work - OhioRise position is a full-time role with a weekly schedule of 40 hours, operating on a day shift. The role is based at the South Campus within the PS Network - OhioRise department. The primary focus is on providing comprehensive social work services, including psychosocial assessments, interventions, resource development, and leadership within the healthcare team. This position emphasizes age-specific and culturally inclusive patient care, interdisciplinary collaboration, and advocacy for patients and their families within the healthcare system and community.

Requirements

  • Master's degree in Social Work or Counseling
  • 5+ years of work experience in a related job discipline.
  • Current independent Social Work (LISW) or LPCC license to practice in the State of Ohio OR LSW with LISW within 1 year of hire

Responsibilities

  • Consistently integrates age-specific and culturally inclusive concepts into patient care, considering both chronological age and developmental functioning.
  • Works for system improvement, promoting patient/family/caregiver well-being.
  • Identifies and advocates for the needs of the patient/families/caregivers within the healthcare system and community.
  • Acts as a liaison between the patient/family/caregiver, the medical team, and outside agencies.
  • Acts as a mediator within the medical team on behalf of the patient/family/caregiver.
  • Empowers the patient/family/caregiver to problem-solve by exploring options of care and alternative plans.
  • Facilitates communication and coordination between healthcare team members while involving the patient/family/caregiver in decision-making.
  • Conducts a comprehensive psychosocial assessment of the patient/family's health care needs to identify strengths, challenges, and opportunities.
  • Evaluates patient/family/caregiver safety concerns and intervenes appropriately to reduce risk.
  • Reports suspicions of abuse and neglect to legally mandated authorities.
  • Utilizes the psychosocial assessment to establish treatment interventions for the comprehensive plan of care.
  • Provides psychosocial services as identified in the patient's comprehensive plan of care.
  • Provides supportive counseling to improve emotional coping and adherence to the treatment plan.
  • Collaborates with referral sources and health care team members.
  • Educates the patient/family/caregiver and health care team members about treatment options.
  • Provides self-management support to high-risk/complex patients/families.
  • Provides groups and programs for special populations as needed.
  • Provides anticipatory guidance and planning to reduce or eliminate psychosocial barriers to discharge.
  • Coordinates and facilitates family-centered care team meetings.
  • Provides crisis assessment and interventions to restore equilibrium.
  • Develops resource networks, excels at resource utilization, and acts as a resource to peers.
  • Identifies needs, develops programs, and evaluates outcomes for special populations, implementing changes based on outcomes.
  • Facilitates collaboration with community agencies to enhance investigation and intervention processes.
  • Advocates for patient populations on a systems level by developing programs and protocols.
  • Serves as a recognized expert to the health care team and/or community on social work practice.
  • Provides work direction, leadership, mentoring, and coaching to assigned personnel.
  • Participates in the recruitment and selection process for candidates.
  • Coordinates work schedules and clinical activities for all clinical staff.
  • Meets routinely with clinical staff to address needs and ensure continuity of care.
  • Provides input for employee annual performance evaluations.
  • Participates in the development of long-term and short-term goals for the location and employees.
  • Works as a liaison between school districts and the division.
  • Attends monthly and quarterly meetings for school districts involving themselves and supervised staff.
  • Ensures clinical practice conforms to local, state, and federal regulations and accrediting standards.
  • Ensures clinical competency of all clinical staff.
  • Responds to, documents, and analyzes complaints associated with therapy services.
  • Maintains the minimum level of continuing education required by the State licensing board.
  • Possesses sound knowledge of procedures and regulations specific to the assessment and care of infants, children, and adolescents.
  • Complies with established Cincinnati Children’s and professional standards.
  • Complies with policies and procedures protecting patient rights for informed consent and handling of confidential information.
  • Reviews timeliness, accuracy, availability, and security of information.
  • Attends required Cincinnati Children’s training sessions.
  • Complies with Infection Control policies and procedures.
  • Provides services in a manner that is regularly and conveniently available to meet patient, family, and employee needs.
