About The Position

The Lead Community Life Coach plays a dual role, focusing on delivering person-centered care and services within a day program setting while also contributing to the program's overall purpose of helping individuals with developmental disabilities achieve their full potential. This role involves developing activities, coordinating programming, and providing guidance to other Community Life Coaches. The Lead Community Life Coach acts as a resource for questions and offers support to the Manager of Day Programs as needed.

Requirements

  • High School degree or equivalent
  • Previous experience in a similar role and responsibility, work, volunteer, community role, or educational setting that focuses on serving and helping others.
  • Must have a valid driver's license
  • Must be able to pass a background check
  • Adult CPR and first aid certification
  • Theoretical or practical understanding of serving others.
  • Proficiency in giving & receiving feedback.
  • Ability to hold self & others accountable.
  • Skills in collaboration.

  • Conflict resolution skills.
  • Proficiency in leveraging technology to the fullest.
  • Ability to achieve quality and continuous improvement of self and processes.

Nice To Haves

  • 6 months or more previous leadership experience preferred (project management, team/task lead, supervisor, etc.)

Responsibilities

  • Deliver person-centered care and services to individuals, following established plans created in collaboration with various stakeholders.
  • Utilize health information technology for timely and comprehensive documentation.
  • Identify community resources and activities, facilitating participation in typical and integrated community events.
  • Accompany participants to activities, if needed, to facilitate involvement and maintain independence.
  • Communicate progress, successes, and challenges to guardians/care providers.
  • Ensure the personal safety of participants and maintain electronic records of progress, incidents, and challenges.
  • Assist with tours and orientations for new clients and their guardians.
  • Provide personal care and support as necessary, including toileting and medication assistance.
  • Track and document all attendance and transportation, providing data to the Program Manager for billing.
  • Revise pickup/drop-off routes as needed and communicate daily route adjustments based on messages and client call-offs.
  • Welcome, onboard, and train new colleagues, identifying additional training and development needs for all staff.
  • Maintain strong working partnerships with colleagues, leadership, PWS, and parents/guardians.
  • Ensure standard operating procedures are implemented consistently, including staff schedules, personal funds, ordering supplies, and monitoring medical supplies.
  • Act as the first line of resource for colleagues, individuals served, and parents/guardians.
  • Bring forward recommendations for quality and process improvements.
  • Provide day-to-day task leadership to Community Life Coaches.
