About The Position

Rushford Center, part of Hartford HealthCare, is a leading provider of addiction and mental health services in Connecticut, recognized nationally for its prevention, treatment, and recovery programs. They offer a wide range of services including Acute Care and Evaluation, Residential Addiction Treatment, Individual, Group & Family Therapy, Intensive Outpatient programs, and Community Support Programs. The Lead Clinician works with interdisciplinary teams to deliver evidence-based clinical services, maintains ethical standards, and assumes leadership responsibilities as assigned. This role may involve clinical oversight of daily operations in the absence of a manager. The position emphasizes patient-centered care, collaboration, and efficient resource utilization within a supportive and growth-oriented healthcare network.

Requirements

  • Master's Degree in Social Work, Counseling, or related field.
  • One year of experience as a licensed clinician.
  • Active Connecticut LCSW, LPC, LMFT license.
  • Valid CT driver's license.

  • Experience in leading clinical care teams is preferred.

Responsibilities

  • Conducts a comprehensive bio-psychosocial assessment and establishes a care plan.
  • Collaborates with the interdisciplinary team to assess appropriate level of care and risk factors.
  • Develops and implements care transition/discharge plans and coordinates consults/referrals.
  • Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan.
  • Monitors progress of interventions and revises plan to meet patient/family needs.
  • Provides education to patient/family regarding goals, disease prevention, health maintenance, and treatment adherence.
  • Provides direct patient care including case management, care coordination, interdisciplinary collaboration, crisis intervention/safety planning, counseling, and emotional support.
  • Refers patients to appropriate levels of care and available resources.
  • Maintains timely, patient-centered documentation in accordance with regulatory requirements.
  • Provides clinical social work interventions to all patient populations.
  • Assists in the coordination of programming, including staff scheduling, clinical activities, program development, utilization review, caseload assignment, and meetings.
  • Assumes other leadership responsibilities and initiatives as directed by the manager, such as assisting in hiring, supporting initiatives, clinician productivity, leading team meetings, and conducting staff education.
  • Participates in administrative and development activities including team meetings, clinical supervision of staff, and staff education.
  • Provides social work field supervision to second-year social work students.
  • Supports the development and efficient execution of hospital and/or unit-based programming.
  • Works as a team member to contribute to improved patient experience.
  • Ensures the cost-effective utilization of resources.
