About The Position

The Lead Clinical Case Management Supervisor will provide case management oversight of referral and intake process of the SIL program providing leadership and training to case management staff working in the field to establish effective relationships to people with mental health issues and meet goals of individual client-centered care plans, which may include service coordination, medication management support, assistance with life skills training, finding and maintaining housing and healthcare. A collaborative and results-oriented leader who will supervise master’s level social work and multi-disciplinary case management staff and manage overall operations of an integrated services program. The Supervisor is responsible for monitoring outcomes toward achieving program and individual outcomes. The Supervisor participates in the middle management team and is responsible for coordinating with other agency departments, hiring and evaluating staff teams, developing resources and implement training to ensure a well-trained high functioning team, and to provide direct oversight of all aspects of program to ensure quality and compliance. The Clinical Supervisor reports to the Clinical Program Director.

Requirements

  • Master’s degree in Psychology, Social Work, Counseling, or a related field & active registration (LCSW, LMFT, LPCC) with the California Board of Behavioral Sciences or Board of Psychology.
  • Direct Service Experience – at least three years of full-time social work or casework employment in the field of family or child welfare services required.
  • A strong dedication to cultural competency that creates conditions that remove barriers and increase a sense of belonging.
  • Three years’ experience working with people with moderate to severe mental health needs, people with histories of homelessness, and/or who have a co-occurring disorder.
  • BHI conducts background checks on all candidates who have received a conditional job offer. Satisfactory completion of background checks is a requirement for employment with the agency.
  • A valid driver’s license, reliable personal vehicle, personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are essential job requirements.

Responsibilities

  • Hire, train, develop and supervise/coach a multi-disciplinary team of social work and case management staff.
  • Lead team members in setting personal and program-wide work goals and in maintaining accountability for goal achievement.

  • Provide training and consultation grounded in evidence-based practice but not limited to trauma-informed care, strength-based approach and motivational

  • Ensure quality of services and accuracy and timeliness of all aspects of participant records including timely submission of all utilization, fidelity and outcome data.
  • Participate in meetings to review participant progress and utilize data to evaluate progress and consider strategies to recalibrate intervention to ensure success.
  • Include development of team-based strategies and excellent collaboration among team members.
  • As assigned, implement intake and referral system to ensure full census, low vacancies, and retention in program.
  • Effectively manage team to ensure baseline and in program assessments, summaries and action plans are timely and accurate.
  • Provide procedural oversight and assign cases to program case managers.
  • Intakes all ACCESS referrals of new clients to the SIL Program.
  • Facilitate weekly and bi-weekly triage meetings to coordinate services.
  • Support team with training to develop effective rapport, case management, crisis intervention, documentation and time management skills.
  • Ensure 52% of activities are linked to billable behavioral health services.
  • Builds, maintains and complies with Bonita House’s policies and procedures as it relates to day-to-day program operations in collaboration with Clinical Program Director, and records maintenance (staff performance evaluations, professional development strategies, training recommendations etc.).
  • Foster relationships with collaborative partners and potential referral sources including Police Departments, Justice System, Behavioral Healthcare Services staff, County Healthcare staff, etc.
  • Develop relationships with the community-at-large to instill support and confidence in the agency and in the program.
  • Serve as a spokesperson for the agency when called upon.
  • Assist in developing materials, assessment tools, and other program processes/documents/tools.
  • Facilitate regular team and case conference meetings.
  • Assist in coaching staff and in ongoing educational efforts.
  • Other duties as assigned by Clinical Program Director.
