The Lead Case Manager provides comprehensive, housing-focused case management for participants in transitional housing programs, including full assessments of physical, mental, and social needs. This role ensures that participants receive the appropriate services aligned with their individualized service plans and that all actions remain compliant with Grants Per Diem (GPD) Program guidelines. The Lead Case Manager also provides leadership to program staff, supports quality assurance, coordinates services, and ensures participant progress toward housing stability.
