About The Position

The primary objective of this position is to manage case management services and staff for LEAD Minneapolis. This person reports to the LEAD Project Manager and is responsible for supervising LEAD’s case management operations and partnering with leaders of comparable programs to foster best practices. The Case Management Supervisor will help hire and supervise LEAD case managers, coordinate services, including program referrals and case assignments, assist the LEAD Project Manager in maintaining a balanced program budget, and develop new partnerships and collaborations with community organizations/businesses to enhance LEAD services. The ideal candidate will be extremely organized, have strong leadership skills, enjoy working in a team, and possess strong attention to detail.

Requirements

Bachelor’s degree in a relevant field, or demonstrably equivalent knowledge gained through alternative courses of study and life experience.
Two years of experience minimum working communities of color and other marginalized communities.
Experience serving populations impacted by extreme poverty, behavioral health conditions, and mental illness from a harm reduction model of care.
Experience supporting and advocating for participants within the criminal justice system including court appearances and written communication.
Ability to work in trauma-informed ways with individuals struggling with mental health, homelessness, chemical dependency, and other risk factors.
Experience supervising and providing constructive feedback to teams.
Skilled at analyzing and interpreting data with the goal of improving service delivery and quality.
Ability to write in English sufficiently to provide correct grammar in grant reports.
Ability to advocate and effectively communicate and problem-solve under pressure in high-stress situations.
Demonstrated ability to keep professional boundaries with program staff.
Ability to operate professionally, manage personal and professional stress, maintain self-care, and demonstrate intrapersonal skills is a must.
Strong organizational, time management, and planning skills with an ability to multi-task is must.
Experience managing and balancing program budgets.
Knowledge of Salesforce or other client data reporting system, running reports, data quality modifying assessments to meet the needs of the populations we serve.
Effective and experienced public speaker with at least one year of experience speaking at events and trainings.
High level understanding of different learning styles. Able to modify training to meet the needs of all team members.
Access to reliable vehicle and ability to maintain good driving record and insurance levels, as directed by the agency, to fulfill job duties that require travel into the community.
Ability to work a flexible schedule that may include evenings and weekends.

Responsibilities

Provide subject matter expertise regarding harm reduction best practices for working with individuals with criminal legal system involvement, behavioral health needs, homelessness, and/or extreme poverty.
Guide content & curriculum development for ensuring LEAD case management staff stay abreast of outreach and case management best practices, with particular emphasis on harm reduction, trauma-informed care, and motivational interviewing.
Programming expert in harm reduction and trauma-informed care.
Stay current with and connected to available community resources.
Coordinate services, including program referrals and case assignments, workload management.
Review client files, assessments, goals, case notes for accuracy and quarterly reporting.
Monitor timeliness and quality of data entry in Salesforce and other databases, ensuring that all client and service information is accurate.
Oversee program compliance with funding contracts and obligations.
Develop and maintain an effective program evaluation framework for the program.
Maintain and build referral network; develop new partnerships and referral strategies.
Collaborate with LEAD’s Project Manager to coordinate service delivery, referrals, program evaluation, etc.
Interview and hire program staff.
Directly supervise program staff and provide weekly supervision.
Provide performance evaluation and management for staff, including goal setting and professional development.
Identify and resolve staffing problems and conflicts.
Facilitate weekly staff meetings.
Provide direction and support staff during challenging client situations and crises, ensuring that service responses are appropriate and effective in addressing client needs.
Monitor ongoing professional development of staff and seek out new opportunities.
Engage in public leadership and policy advocacy around the issues surrounding racial justice, chemical health, commercial sexual exploitation, trafficking, or prostitution.
Maintain positive and regular contact with current partners and funders, as well as with significant program stakeholders.
Develop new partnerships and collaborations with community organizations/businesses to enhance LEAD services.
Work with LEAD Project Manager to develop and maintain a balanced program budget.
Plan & manage program expenses, including authorizing expenses and assuming responsibility for program spending.
Use resources wisely.
Help maximize resources and leverage opportunities and develop new financial resources.
Familiarize yourself with Pillsbury United Communities’ policies on fiscal responsibility and ensure that all staff abide by those policies.
