About The Position

Heritage Health Network is hiring a Lead Care Manager to support members through its Enhanced Care Management (ECM) program. This hybrid role combines two remote workdays with three days in the field, building in-person connections with members across San Diego County. The position focuses on coordinating member care, developing personalized care plans, and helping members access appropriate healthcare and community resources.

Requirements

• Experience in care or case management; community-based experience is preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.

Nice To Haves

• Knowledge of Medi-Cal and Enhanced Care Management (ECM) is strongly preferred.
• Bilingual English/Spanish skills are preferred.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify and address gaps in care.
• Maintain accurate, timely documentation.
