About The Position

Sycamores provides a spectrum of equitable mental health services, housing support services, and coordinated care management through various programs to multicultural populations of all acuities. Although it serves a variety of consumers, the Agency focuses primarily on providing care to low-income, high-needs, at-risk children, youth, families, and adults. The role of the Lead Care Manager (LCM) II is to provide support, coordinate care and provide linkage to Managed Care Members within the home and/or in the community. These interventions are provided to increase skills and overall functioning levels of Members developed through an individualized, Member-centered process. Additionally, this is a position that is focused on whole-person care, wellness, and building resilience through interventions, linkages, and referrals to appropriate community and partner resources.

Requirements

  • High school diploma or AA degree with one year of direct care experience required.
  • Bachelor’s degree plus two years of direct care experience preferred.
  • Successful completion of certified care coordination (i.e., UCSF Health Coaching and Complex Care Coordination).
  • Successful completion of specialized Program Projects (i.e., co-location placement).
  • Maintains all required licenses and certifications.
  • Personal, reliable vehicle for travel to various locations.
  • Valid California driver’s license.
  • Acceptable driving record as determined Agency policy.
  • Proof of auto insurance meeting minimum coverage requirements.
  • Ability to work independently with a high level of integrity.
  • Ability to work a non-traditional work schedule to meet Member needs.
  • Ability to travel and work in the community and in the home environment.
  • Have strong computer and typing skills to complete necessary documentation.
  • Must satisfy a pre-employment background check, drug, and health screen.

Nice To Haves

  • Two years of direct care experience preferred.
  • Bilingual capacity preferred.
  • Experience in Homeless Services, Housing Navigation, Physical Health Navigation, Residential Facility, Group Home, Child Welfare or Juvenile Justice is highly desired.
  • Knowledge of resources in LA County Service Planning Areas, a plus.
