About The Position

Humana’s Primary Care Organization is one of the largest and fastest growing value-based care, senior-focused primary care providers in the country, operating multiple centers across eight states under two brands: CenterWell & Conviva. Operating as a payor-agnostic, wholly owned subsidiary of Humana, our centers put the unique needs of seniors at the center of everything we do. Our Clinics offer a team-based care model where our physicians lead a multi-disciplinary care team supporting patient’s physical, emotional, and social wellness. The Cardiology Lead specializes in diseases of the heart and blood vessels and is the Cardiology program leader, and part of a clinical care team that focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention for senior patients. The Cardiology Lead manages complex cardiac conditions such as abnormal heartbeat rhythms, and congestive heart failure, as well as provides preventative techniques for dealing with potential heart-related illnesses. The Cardiology Lead spends approximately 80% of their time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, etc.) as it relates to direct patient care, and approximately 20% of their time leading the program. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, uses independent judgment requiring analysis of variable factors and determining the best course of action.

Requirements

  • Current and unrestricted medical license or willing to obtain a medical licenses in state of practice
  • Graduate of accredited MD or DO program of accredited university
  • Excellent verbal and written communication skills
  • Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients
  • Fully engaged in the concept of “Integrated team based care” model
  • Willingness and ability to learn/adapt to practice in a value-based care setting
  • Willingness to drive between two to four centers in personal vehicle
  • Board Certification or Eligible to become certified (ABMS or AOA) in Cardiovascular Disease

Nice To Haves

  • Board Certification in echocardiography, nuclear cardiology, and vascular ultrasound (RPVI)

Responsibilities
