About The Position
Hometown Health Center is expanding its Behavioral Health team and is seeking a Therapist (LCPC, LSCW) to join our team. Hometown Health Center is a Federally Qualified Health Center located in rural central Maine. We provide comprehensive health care including medical, dental, and behavioral health services. This position provides behavioral health counseling to patients and their families. Our behavioral health professionals work collaboratively with an interdisciplinary team to develop appropriate supports and interventions, discharge planning and service coordination. Requirements
- LCSW or LCPC licensed by the State of Maine (conditional license considered)
Responsibilities
- Provide behavioral health counseling to patients and their families.
- Work collaboratively with an interdisciplinary team to develop appropriate supports and interventions.
- Engage in discharge planning and service coordination.
