About The Position

As a Patient Education Programs Educator, you will play a key role in empowering Chronic Kidney Disease (CKD) patients with knowledge about their health and treatment options. In this divisional role, you will bridge virtual engagement with hands-on regional support, traveling across your assigned territory to deliver direct education and partner with local care teams.

Requirements

• Must be able to travel across assigned regions.

• Must be able to manage data entry in program management systems.
• Must be able to deliver education on CKD management and dialysis modalities.
• Must be able to provide Spanish-language educational support.

• Familiarity with Chronic Kidney Disease (CKD) and dialysis modalities.
• Proficiency in Spanish.

Responsibilities

• Educate & Guide Patients: Deliver engaging virtual and in-person education on CKD management and all dialysis modalities, including Home Hemodialysis (HHD), Peritoneal Dialysis (PD), in-center hemodialysis, and kidney transplantation.
• Travel Within the Division: Travel across assigned regions to conduct live patient sessions, boost program capacity, and support field teams in expanding class outreach.
• Connect Telephonically: Provide direct phone support to schedule participants, answer questions, follow up on care pathways, and offer targeted Spanish-language educational support.
• Support Program Operations: Keep participant records accurate by logging tasks, tracking pipeline opportunities, and managing data entry in our program management systems.
