About The Position

JOB SUMMARY The Job Coach is responsible for on-site job training, mentoring, coaching, and workplace assessment services for individuals in the community.

Requirements

  • Excellent written and verbal communication skills.
  • Excellent organizational and time management skills.
  • Proficient in Microsoft Office Suite or similar software.
  • Flexibility to ever-changing schedules and shifts.
  • Strong ability to problem-solve.
  • High School Diploma
  • 1-3 years' experience in related field preferred

  • Experience with developmentally disabled population preferred

Responsibilities

  • Provide coaching and training to actively working Vocational Rehab clients.
  • Provide coaching and on-going training to clients receiving Extended Services through Waiver and Community Support budgets as outlined in their budgets.
  • Assist with the development of natural supports at the worksite.
  • Monitor and document client progress on struggles and successes to maintain established goals.
  • Maintain necessary documentation, data and reports as required by CARF (Commission of Accreditation Rehabilitation Facilities) standards, funding sources and agency.
  • Maintain necessary documentation daily, weekly, and monthly to adhere to department standards.
  • Advocate for and pursue services appropriate to meet the individual needs of clients.
  • Maintain the required productivity level at 85% client specific productivity hours weekly based on caseload of active job coaching and follow-along.
  • Develop and maintain productive relationships with local employers, referral sources, clients, parents, co-workers, and others as appropriate.
  • Become familiar with client specific information by attending client conferences and reading files prior to onset of coaching services, including but not limited to BSP (Behavior Support Plan) training.
  • Attend and participate in department, division, and agency meetings, and training required.
  • Maintain awareness of local, state, and national trends that impact employment services.
  • Demonstrate appropriate professionalism and be a role model for team members and clients.
  • Assist and Provide Discovery Services as needed and assigned.
  • Your caseload will likely require working in Porter, Lake, and surrounding counties as well as various shifts including nights and weekends.
  • All other duties as assigned.
