About The Position

IRIS Consultant Connections – An IRIS Consultant Agency (ICA) by Lutheran Social Services of Wisconsin & Upper Michigan (LSS) Location: Oconto & Marinette Counties, WI Work Arrangement: Remote/Field-Based with local travel Are you passionate about helping people live meaningful, self-directed lives? As an IRIS Consultant (IC) , you'll partner with older adults and individuals living with disabilities to develop personalized plans that support their goals, independence, and quality of life. This role combines relationship-building, advocacy, problem-solving, and community engagement to empower participants through Wisconsin's IRIS (Include, Respect, I Self-Direct) program. In addition to supporting participants, you'll have the opportunity to contribute to a collective leadership team that values trust, collaboration, innovation, and shared decision-making.

Requirements

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Nursing, Special Education, or a related field and at least one year of supervised experience working with older adults and/or individuals with disabilities; OR Four years of direct experience providing social services to older adults and/or individuals with disabilities in community-based settings; OR Current IRIS Consultant in good standing with a Wisconsin Department of Health Services exemption.
  • Valid driver's license and reliable personal vehicle
  • Acceptable driving record
  • Ability to travel within Oconto and Marinette Counties (approximately 30–50%)
  • Ability to pass required caregiver background checks
  • Passion for person-centered services and self-directed care.
  • Ability to work independently and collaboratively.
  • Strong communication, relationship-building, and problem-solving skills.
  • Knowledge of community resources and human services.
  • Proficiency using technology including smartphones, tablets, and computer systems.
  • Strong organizational and documentation skills.

Nice To Haves

  • Occasional overnight travel may be required

Responsibilities

  • Build trusting relationships with IRIS participants and those important in their lives.
  • Support participants in understanding and maximizing IRIS services and resources.
  • Develop and maintain person-centered IRIS Support and Services Plans.
  • Assist participants in obtaining and maintaining funding for long-term care supports.
  • Connect individuals to resources that promote independence, inclusion, and community engagement.
  • Maintain accurate documentation and ensure compliance with program requirements.
  • Participate in peer mentoring, team collaboration, hiring activities, and ongoing learning.
  • Strengthen community connections and support networks for those served.
  • May conduct Long-Term Care Functional Screens.
