To provide a single point of focus for rehabilitation patients as they move throughout the continuum of care to assure smooth, appropriate, coordinated, timely and resource sensitive transitions and assuring those services provided are in the best interest of the patient and family. This position operates independently, working cooperatively and collaboratively with patient, families’ health care providers, payors, and management. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill. This position requires self-direction, motivation, organization, and interpretation of information relating to a wide variety of policies, procedures, and programs. Work situations are frequently non-routine, requiring analysis and exhibiting independence and the use of discretion and good judgment in decision making. Internal contacts are at all levels and require establishing rapport and developing strong working relationships. External contacts include outside organizations, physicians.
