About The Position

Lehigh Valley Health Network (LVHN) is seeking a Case Manager Psychiatry to provide mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members. The role involves coordinating care for clients, assisting them in accessing services and resources to maintain them in the community in the least restrictive environment. Requirements

Bachelor’s Degree in Social Work, Psychology, or related field and actively enrolled in a related Master's degree program.
Ability to remain patient and calm in stressful situations.
Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.
Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive care plan.

Responsibilities

Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
Utilizes management activity to include authorization of services as well as management of denial and appeal process.
Provides case management and care coordination services for patients.
Intercedes on behalf of individual patients to assist them in achieving and maintaining community stability.
Acts as a liaison between patients, referral sources, and funders.
Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.
Manages all elements of mental health voluntary and involuntary commitments processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs).
