About The Position

The Internist is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations. The Internist will provide examinations, review laboratory, referral, and imaging results to determine diagnoses, and document visits accurately using appropriate templates. The position requires managing both common and complex illnesses in adults and the elderly, incorporating health promotion, disease prevention, and a holistic healthcare model. Chronic disease management skills are essential, utilizing a team-based, patient-centered approach. The Internist will act as a diagnostician, therapist, counselor, and coordinator of care, with a focus on disease prevention, wellness, substance abuse, mental health, and understanding the diagnosis and treatment of cancer, infections, and diseases affecting internal organs. The role also emphasizes providing excellent customer service, communicating courteously with patients and staff, managing wait times, and educating patients on health literacy. Accountable for patient status as measured by relevant clinical quality measures, the Internist will promote and participate in daily huddles, incorporate ancillary health team members in patient care, and set and monitor self-management plans. Responsibilities include assisting in the assurance of clinical procedures, maintenance of clinical protocols, chart and peer review, performance improvement efforts, staff education, and FHCN promotion. The role may involve preceptorship of mid-level practitioners and participation in the rotation of medical students and residents. Participation in Network Health Fairs, community events, site and provider meetings, and meeting Network credentialing expectations are also required. Adherence to the Attendance and Absenteeism Policy and the ability to work at any FHCN location are essential.

Requirements

  • Completion of a Doctoral-level degree program with a recognized major and a minimum cumulative GPA of 2.5; or If an individual has completed a degree at a higher level than required for the role and had a stronger GPA in that program, they may provide proof of GPA from that degree in lieu of the Master's degree.
  • Minimum credit score of 650.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed in the state of California as an Internist.
  • Effective communication of professional opinions and extrapolations of information collected and synthesized/analyzed.
  • Determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  • Preparation and execution of presentations to large groups.
  • Regularly required to sit and use repetitive hand movement to type and grasp.
  • Frequently required to stand or walk.
  • Occasionally lift and/or move up to 20 pounds.
  • Meet Network and credentialing expectations for licensure, and employment.
  • Adhere to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.

Responsibilities

  • Direct provision of primary care medical services meeting all established productivity expectations.
  • Provide examination and review appropriate laboratory, referral and imaging results to determine a diagnosis.
  • Document visits accurately utilizing appropriate templates and records relevant data to attain a diagnosis.
  • Attain budgeted visits.
  • Meet clinical care measures.
  • Lock records in the 72-hour window.
  • Provide comprehensive, continuous, and coordinated care managing both common and complex illness adults, and the elderly.
  • Provide comprehensive health care including health promotion and maintenance, prevention of illness and disability, and application of a holistic healthcare model which addresses both protective factors and risk factors affecting the health and wellbeing of patients.
  • Utilize chronic disease management skills to provide a team-based, patient-centered approach to the planned care of chronic conditions with the goals of maximizing quality of life and health care.
  • Act as a diagnostician, a therapist, a counselor, and a coordinator of care.
  • Understand the diagnosis and treatment of cancer, infections, and diseases affecting the internal organs with incorporation of essentials of primary care internal medicine with focus of disease prevention, wellness, substance abuse, mental health, and effective approach to both health beliefs and comprehensive care needs of patients.
  • Provide excellent customer service and care.
  • Consistently behave with courtesy when interacting with patients/ family members, support staff, and Network staff.
  • Remain aware of wait time during the daily workflow.
  • Provide the patient with educational materials and resources appropriate to the patient’s health literacy needs.
  • Achieve patient status as measured by relevant clinical quality measures.
  • Promote and participate in daily huddles.
  • Incorporate the participation of ancillary health team members in managing the care of patients / families.
  • Set and monitor self-management plans.
  • Assist in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
  • Complete chart and peer review responsibility in a timely manner.
  • Assist the patient care team with performance improvement efforts, staff education, and FHCN promotion.
  • Assist the patient care team with performance evaluations and quality improvement.
  • Assist in the preceptorship of mid-level practitioners.
  • Participate in rotation of medical, physician assistant students, and dental residents where relevant.
  • Participate in Network Health Fairs & other community events.
  • Participate in site & provider meetings in a meaningful way.
  • Meet Network and credentialing expectations for licensure, and employment.
  • Adhere to the Attendance and Absenteeism Policy.
  • Present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Perform other duties as assigned.
