The Internist is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves direct provision of primary care medical services, meeting all established productivity expectations, including examination, reviewing laboratory and imaging results to determine a diagnosis, and accurately documenting visits. The Internist will manage common and complex illnesses in adults and the elderly, providing comprehensive healthcare that includes health promotion, maintenance, illness prevention, and a holistic approach to patient well-being. Chronic disease management is a key component, utilizing a team-based, patient-centered approach. The role requires understanding the diagnosis and treatment of various diseases, with a focus on disease prevention, wellness, substance abuse, mental health, and effective patient care. Additionally, the Internist is accountable for providing excellent customer service, managing patient status through clinical quality measures, and participating in performance improvement efforts and staff education.
