Requirements

• MD or DO
• Board Certified or Board eligible
• Active/Unrestricted medical license
• DEA Controlled Substances Registration Required
• Licensed Physician Required
• Drug Enforcement Administration (DEA) License Required
• Over 5 years Internal Medicine Experience
• Over 5 years Family Practice Experience

Nice To Haves

• Basic Life Support (BLS) Preferred
