About The Position

Mindify is a patient-centered healthcare organization providing medical and behavioral health services to diverse communities throughout New Jersey. Our approach focuses on coordinated care, prevention, chronic disease management, and clear communication with patients and their families. Mindify is seeking an experienced Internal Medicine Physician, Nurse Practitioner, or Physician Assistant to provide primary care services to adults at our Fair Lawn location. Fluency in both English and Spanish is required for this position. The provider must be comfortable conducting appointments, discussing diagnoses and medications, and explaining treatment plans in both languages. The position focuses on preventive care, acute medical concerns, chronic disease management, and patient education. The provider will also coordinate care with specialists, behavioral health clinicians, and other members of the healthcare team.

Requirements

  • MD or DO degree from an accredited medical school OR Licensed Nurse Practitioner with primary care or internal medicine experience OR Licensed Physician Assistant with primary care or internal medicine experience
  • Active and unrestricted New Jersey professional license
  • Board certification or board eligibility in Internal Medicine for physician applicants
  • Current DEA registration and applicable prescriptive authority
  • Professional fluency in spoken and written English and Spanish
  • Ability to conduct complete patient visits in either language without an interpreter
  • Strong clinical assessment, diagnostic, and patient-management skills
  • Experience using an EMR
  • Strong interpersonal and communication skills

Nice To Haves

  • At least two years of outpatient internal medicine or primary care experience
  • Experience working with diverse or underserved populations
  • Familiarity with value-based care and quality measures
  • Current BLS and ACLS certifications

Responsibilities

  • Conduct medical evaluations and provide diagnosis and treatment for adult patients
  • Manage acute and chronic conditions, including hypertension, diabetes, and cardiovascular disease
  • Perform annual physicals, wellness visits, and preventive screenings
  • Develop and update individualized treatment plans
  • Order and interpret laboratory tests, imaging, and other diagnostic studies
  • Communicate test results and follow-up recommendations to patients
  • Prescribe medications and monitor effectiveness and side effects
  • Educate patients about their conditions, medications, preventive care, and lifestyle changes
  • Document patient encounters accurately and on time in the EMR
  • Coordinate care with specialists, behavioral health providers, and other healthcare professionals
  • Participate in clinical meetings and quality-improvement activities
  • Follow federal, state, and organizational requirements
  • Provide care in both English and Spanish
