About The Position

Under the direct supervision of the Senior PACE Program Director, the Interdisciplinary Operations Manager is responsible for standardization and implementation of workflows, training, coaching, and identifying areas of improvement within the IDT Moderator role. This role encompasses training, coaching, ongoing monitoring of key performance indicators, EPIC optimization, and ensuring compliance with all PACE regulatory requirements. The role also involves collaborating with leadership to develop workflows, training programs, and EPIC optimizations that align with PACE regulations and policies, with a strong focus on enhancing patient satisfaction. The manager is responsible for interviewing and selecting IDT Moderators, onboarding and training new moderators, and supporting leadership in optimizing training materials. They ensure team members effectively fulfill their roles, create and maintain methods for monitoring compliance and effectiveness, and monitor/report key performance indicators. The role involves communicating operational issues to the Center Manager for continual program improvement, optimizing reporting for leadership oversight, and collaborating with site leadership for standardization and efficiency. The manager also assists with annual evaluations, meets regularly with Site Day Leadership and Technical Managers, and ensures team members understand their roles and the importance of teamwork. They coordinate orientation and in-service programs, ensure quality service within standards and financial targets, and oversee resource utilization. Participation in team development, troubleshooting, advising, teaching, coordinating, and delivering care are key. Collaboration with QAPI for regulatory activities and compliance measures, including quality improvement projects, is essential. The manager supports the PACE Center Manager and Medical Director in monitoring intake, disenrollment, and panel management, and may be required to fulfill an IDT Moderator role. Professional growth is encouraged through educational programs and literature. Creative thinking, problem-solving, and empowerment are fostered to improve morale and teamwork. Close work with Directors and other Managers/Supervisors is expected, along with excellent organizational skills and the ability to follow up on tasks. Other related duties are assigned as requested.

Requirements

  • Bachelor’s degree in healthcare or related field or equivalent relevant experience.
  • Minimum of 2 years experience of working with an Interdisciplinary Team.
  • Minimum of 2 years experience working with the frail elderly population.
  • Good oral and written communication skills.
  • Verbal ability for an accurate and tactful explanation.
  • Skills necessary to provide care for geriatric patients.
  • Thrive in a team environment.
  • Great organizational and supervisory skills.
  • Ability to effectively handle difficult and unusual interpersonal situations.
  • Only acts within the scope of his or her authority to practice.
  • Meet a standardized set of competencies for the specific position description established by SD PACE and approved by CMS before working independently.
  • Knowledge of medical terminology.
  • Working knowledge of Microsoft Windows, MS-Office.
  • Knowledge of health programs and eligibility, coding, and healthcare compliance and medical records.

Responsibilities

  • Standardization and implementation of workflows, training, and coaching for IDT Moderators.
  • Identifying areas of improvement within the IDT Moderator role.
  • Training, coaching, and ongoing monitoring of key performance indicators.
  • EPIC optimization and ensuring compliance with PACE regulatory requirements.
  • Collaborating with leadership to develop workflows, training programs, and EPIC optimizations.
  • Interviewing and selecting IDT Moderators.
  • Onboarding and training new IDT Moderators.
  • Supporting leadership in optimizing training materials.
  • Ensuring all team members effectively fulfill their assigned IDT Moderator roles.
  • Creating and maintaining methods of monitoring key IDT compliance areas and overall effectiveness of IDT Moderators.
  • Monitoring and reporting key performance indicators regularly to leadership.
  • Communicating operational issues and concerns to the Center Manager.
  • Optimizing reporting to increase leadership’s oversight of key areas of the IDT and its services.
  • Ensuring standardization and efficiency of all IDT Moderators.
  • Assisting site leadership in the annual evaluation process for all IDT Moderators.
  • Meeting regularly with Site Day Leadership and Technical Managers to review goals, trends, and performance measures.
  • Ensuring that team members understand the importance of their and others’ roles on the team and remain open to the cooperative dynamic of the team.
  • Emphasizing teamwork and collaboration in all Interdisciplinary and cross-team interactions.
  • Coordinating orientation and in-service programs for staff.
  • Ensuring the provision of quality service within expected standards and financial targets.
  • Ensuring appropriate utilization of organizational resources.
  • Participating in the continued development of the team to improve the team decision-making process.
  • Providing support to the team to troubleshoot, advise, teach, coordinate and deliver care and service to participants.
  • Partnering and collaborating with the QAPI in support of all regulatory activities and compliance measures.
  • Supporting the PACE Center Manager and Medical Director in monitoring intake, disenrollment, and panel management activities.
  • Fulfilling an IDT Moderator role if necessary.
  • Enhancing professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Encouraging creative thinking, problem-solving, and empowerment as part of the facility management group.
  • Working closely with the Directors and other Managers and Supervisors.
  • Performing other related duties as assigned or requested.
