About The Position

The Intensive Home Service Provider (IHSP) provides an intensive form of case management that facilitates care coordination for children and youth in foster care or Juvenile Justice System, or at risk of out-of-home placement. The IHSP provides skill building sessions directly to clients as well as parent coaching. IHSP's facilitate Child and Family Team (CFT) meetings and supports the ICC in necessary tasks related to stabilizing a client's symptoms and improving their functioning.

Requirements

  • Bachelor's degree in social work or a related field; or equivalent education, training, and experience.
  • One year or more of case management and rehabilitation services experience working with parents and youth.
  • Knowledge of community and county resources to facilitate referrals and coordination with appropriate agencies.
  • Knowledge of trauma informed care.
  • Knowledge of mandated reporter laws in California.
  • Knowledge of crisis de-escalation interventions.
  • Proficient writing skills.
  • Proficiency in usage of basic technological tools including laptops/computers, email, phones, and internet required.
  • Working knowledge of Microsoft Office Suite required.
  • Ability to maintain a valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability.
  • Verbal and written fluency in English required

Nice To Haves

  • Knowledge of the child welfare system and juvenile probation system preferred.
  • Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese) desired but not required.

Responsibilities

  • Coordinate care for clients with a multidisciplinary clinical team and outside agencies including developing effective working relationships with agencies and organizations to advocate for clients.
  • Work collaboratively with the Intensive Care Coordinator assigned to the case to assist in monitoring progress for effectiveness of interventions and make decisions regarding the need to adjust treatment or find additional resources and supports.
  • Provide collaborative, culturally sensitive, strengths based, client centered individual/group/family rehabilitation services to youth and their families.
  • Complete all documentation per Medi-Cal, Santa Clara County and AACI standards within expected deadlines.
  • Schedule, facilitate and participate in Child and Family Team (CFT) meetings.
  • Facilitate referral linkages to community resources and perform case management services.
  • Participate in clinical supervision, consultations, trainings, and team meetings.
  • Assist clients with the development of social and community support systems.
  • Complete other related duties and projects as assigned.
